See http://cphs.berkeley.edu to link to current CPHS forms needed for all types of applications to the Committee for Protection of Human Subjects, as well as instructions, guidelines, and further information regarding each form. Definition of CPHS Researcher Titles and Related Responsibilities Before filling out CPHS application forms, please read the following explanation of researcher titles, based on the UC Research Policies & Guidelines, UC Contract & Grant Manual, and UCB Research Regulations. Lead Investigator signifies eligibility to perform research involving human subjects at UC Berkeley. Individuals serving as Lead Investigator must have a formal affiliation (i.e., a faculty or staff appointment or enrolled as a student) with UCB. The type of appointment an individual has determines whether they may serve as Lead Investigator on their own or require a Principal Investigator or Faculty Advisor to sponsor them (see below). For example, a UCB undergraduate or graduate student may serve as Lead Investigator, but must have a Faculty Advisor who also takes responsibility for the conduct of the study and signs off on the CPHS application. Principal Investigator (PI) is an employee of UCB (usually with an academic appointment) who is eligible under University policy to submit proposals for extramural support of a research, training, or public service projects, and to perform research involving human subjects. A PI should serve as Lead Investigator on any UCB study as long as s/he has primary responsibility for the scientific, technical, and administrative conduct of the project. NOTE: Only those with PI status may sign off on the CPHS Core Grant/Training Program application. Faculty Advisor/Faculty Sponsor signifies one who is eligible under University policy to submit proposals for extramural support of research, training, or public service projects, and to perform research involving human subjects. Students or other individuals who wish to perform human research at UCB but do not qualify to serve as Lead Investigator on their own must obtain sponsorship of a faculty advisor or faculty sponsor. Key Personnel includes any individual who contributes in a substantive way to the design, conduct, and/or analysis of the data of a study at or on behalf of UCB. In particular, researchers and staff involved in obtaining informed consent from subjects are considered key personnel.
