INSTRUCTIONS: Study Closure Form

• This form is intended for use when an approved CPHS study has been completed, OR when a study is being closed or an application withdrawn for other reasons.

• If study closure is due to completion, copies of any pertinent publications resulting from the research are requested for OPHS files.

• At least one copy of this agreement should bear the original signature of the Lead/Principal Investigator, and the Faculty Advisor if applicable. Signatures should be legible.
